Cancellation applies to next cycle only

Administrative Cancellation Fee / Refund Limits

$99 fee if canceled after payment but before prescription processing

No refund once prescription is processed

Minimum terms and installment plans remain fully enforceable.

Delays do not pause payment obligations.

Financing Plan Changes

Must request within 7 business days

Remaining balance may be charged without finance fees

Courtesy Communications

Payment Default Policy

Collections may begin after 30 days

Services may be suspended or terminated

Adequate funds

Receiving notices

Unpaid accounts may be sent to collections.

Collection fees

Outstanding balances

Child-Resistant Packaging

Medication may not be shipped in child-resistant packaging.

You are responsible for safe storage.

Telehealth Consent

Telehealth involves risks and limitations.

You must agree to a Telehealth Consent before services.

Lonvya may be a third-party beneficiary of that consent.

Connectivity & Information Responsibility

You are responsible for:

Internet access

Any related costs

You must provide accurate information at all times.

Protected Health Information (HIPAA)

Lonvya is not necessarily a covered entity

Providers/pharmacies may be

Lonvya may act as a business associate

PHI is governed by HIPAA where applicable.
Non-PHI is governed by the Privacy Policy.

Jurisdiction (California)

These Terms are governed by:

U.S. federal law

Arbitration (AAA) – No Class Actions

All disputes must be resolved through binding arbitration:
